Liability Releases
All entries must fill out and sign TWO different waivers.
These forms are linked below.
You can print and fill out by hand the old fashioned way, or key the data in online.
To key in an online form:
-Key in forms work best from a computer - not a mobile device. To key in forms on a mobile device, scroll down for instructions.
-Electronic signatures (made by simply, typing your name on the signature lines) are valid provided you check the “consent box” on each form.
-If you choose Electronic Signature, once complete go File > Export to PDF > Save to your device > Email or Print.
-All completed waivers can be e-mailed in to jody@konatournaments.com, submitted by snail mail or hand delivered.

Instructions To Key In Waivers on Mobile Devices
For those who are filling out our key-in forms on their Androids, utilizing Google Drive (the app), might be the easiest by using their “Edit” > “Form Filling” tool.
For those who are filling out our key-in forms on their iPhones, click here for some helpful steps on the easiest way to get the forms signed and returned back to us!
