Liability Releases:

All entries must fill out and sign TWO different waivers– a “Participant Liability and Publicity Release,” and a “Liability Release & Covenant Not to Sue.”

These forms are linked below. You have a choice of either printable paper PDF forms, or online key-in forms.

If you prefer to key in an online form:

-For optimal results we recommend that you do so from a computer and not a mobile device. If you decide to fill out the forms on-the-go, we do have directions at the bottom of this page to make things easier for you; just scroll down!

-Electronic signatures (made by simply, typing your name on the signature lines) are valid and all that we require.

-All completed waivers can be e-mailed in to jody@konatournaments.com, submitted by snail mail or hand delivered.
