Team Entry Forms are required for ALL entries.

NOTE: Forms can be amended as needed. If you need to update previously submitted Team Entry Form information, please do NOT submit another form, but rather email jody@konatournaments.com with your name, boat name, and the changes you would like to make.

NOTE: Team Base Entry Fees for all tournaments covers up to 4 Anglers and 2 Crew. Any additional team members can be added on for a fee of $50. If you have paid for extra team members, please email their information to jody@konatournaments.com.
